In series 1, we discussed the system challenges and collaborative solutions, sharing best practice and evidenced-based ways of working that can help integrated care boards, trusts and local authorities make meaningful progress on their goals.

Clinical Case Management
An innovative partnership approach to supporting complex discharges and reducing out of area placements.
This session explored how a coordinated, multiagency approach can help safely progress complex discharges and reduce unnecessary delays. We outlined how dedicated clinical case management can improve communication between inpatient teams and community partners, ensuring people receive the right support at the right time. Attendees also heard how this model can reduce out of area placements while improving experience and continuity of care for people with the most complex needs.

Specialist Accommodation Services
A collaborative model for enabling step‑down pathways and improving transitions from inpatient care.
This session outlined how specialist accommodation can create safe and supportive stepdown pathways for people leaving inpatient settings, helping to improve flow and reduce delayed discharges. We explored collaborative models that bring together clinical teams, social care, and community support to enable smoother transitions and more stable outcomes. The session also highlighted how these services can enhance people’s recovery, reduce the risk of readmission, and relieve pressure on acute and longer stay wards.

Crisis Accommodation Services
A responsive, community-based approach to preventing admissions and supporting people in acute distress.
This webinar showcased how community-based crisis accommodation can offer a timely alternative to hospital admission for people experiencing acute distress. We discussed how these services work alongside crisis teams, emergency pathways, and social care to provide rapid, person-centred support that prioritises safety and deescalation. Attendees heard how such models can ease demand on inpatient beds, improve people’s experiences during crisis, and support more compassionate, responsive local pathways.
