The Patient Safety Incident Response Framework (PSIRF) is a framework provided by the NHS for organisations to follow to effectively respond to Patient Safety Incidents. It focuses on systems, processes-in place and learnings to improve patient safety.

“An incident is an unexpected event that has caused harm or had the potential to cause harm to a service user”

The 4 key aims of PSIRF are:

  1. Compassionate engagement and involvement of those affected by patient safety incidents.
  2. Using a range of system-based approaches to learning.
  3. Ensuring responses are considered and proportionate.
  4. Providing a supportive oversight, focused on strengthening response system functioning and improvement.
