In this final session of the 10 Year Plan webinar series, we’ll look at the shift from ‘Analogue to Digital’, exploring how the Hope Haven partnership (one of the 24/7 mental health neighbourhood hub pilots), developed a Multi-Agency Shared Patient Record to address one of the biggest barriers to integration: fragmented systems and lack of shared visibility.
Bringing together leaders from Everyturn, Cumbria Northumberland Tyne & Wear NHS Foundation Trust (CNTW), and The Access Group (TAG), this session will take you through the full journey:
