Our specialist team provides independent clinical assessment, care planning, coordination and review, helping people receive safe, effective and person-centred support in the most appropriate, least restrictive setting for their needs.
Independent of both health and social care providers, we act as a bridge between partners, bring people together to navigate complex systems, coordinate decisions and overcome organisational barriers. This means care can be shaped around each person’s needs, strengths and aspirations, rather than service boundaries, helping to reduce delays and unnecessary bureaucracy.

We work with people, families, clinical teams, commissioners and social care partners to understand long-term needs and develop personalised care and support plans. Our team helps remove barriers to discharge by coordinating access to appropriate funding, identifying suitable accommodation and finding providers that can meet each person’s clinical, practical and social needs. We support timely, safe discharge into sustainable community placements.
For people already living in community placements, we provide ongoing clinical oversight and regular reviews to ensure their care remains safe, effective and appropriate. We identify and respond to changing needs, coordinate adjustments to care packages and work with the person, their support provider, families and statutory partners to maintain stability, prevent placement breakdown and support continued care in the least restrictive setting.
