Specialist dementia assessment & care
We support people at every stage of their dementia care, from early assessment and diagnosis through to ongoing treatment, care, recovery and longer-term support.
Our clinically led multidisciplinary teams, which include 24/7 nursing support within our specialist residential services, provide compassionate care, practical support and timely interventions that promote wellbeing, independence and quality of life.
By working closely with NHS partners, local authorities, families and communities, we provide safe, high-quality, joined-up care that can adapt as a person’s needs change, helping them to live well with dementia.

Our dementia assessment services provide safe, specialist support for people living with dementia whose needs require specialist assessment, care and support. Through comprehensive assessment, observation and personalised care planning, we work alongside health and social care professionals, families and carers to understand each person’s needs, and identify the right long-term support.
Together, we help people move into care that is appropriate and centred around what they need, and help find the right future placement.
Our dementia respite services provide short-term, high-quality care for people living with dementia, while giving families and carers the opportunity to take a planned break. Working closely with health and social care teams, we tailor each stay to the person’s needs, promote wellbeing and support as much independence as possible.
Our specialist dementia accommodation services provide safe, structured and therapeutic environments for people living with dementia who have more complex needs, including people experiencing distress or changes in behaviour that may signal unmet clinical, psychological, social or environmental needs.
Our highly trained teams take time to understand each person, delivering person-centred care that focuses on understanding their history, preferences and unmet needs. We use this understanding to develop personalised support that promotes wellbeing, protects dignity, while helping each person have the best possible quality of life.
Working closely with health and social care professionals, families and carers, we provide the right level of ongoing support and adapt our approach as needs change. Together, we help people achieve the best possible outcomes in an environment that can support their longer-term needs.
Our community behaviour support service provides specialist outreach support for people living with dementia in their own homes and care home settings when they’re experiencing distress or changes in behaviours that may be challenging.
Working closely with people, families, carers and support providers, our experienced teams take time to understand what may be contributing to someone’s distress or changes in behaviour. We then develop practical, person-centred approaches that help reduce distress and improve wellbeing.
Through early intervention, advice, training and ongoing support, we help people stay safely in their communities for longer. This can prevent avoidable hospital admissions and ease pressure on families, carers and care providers.
